1. Please ensure all mandatory fields are completed, and indicate with an "*" the type of claim you are sending (shortage, overage, damage, return).

2. Incomplete or Incorrect forms will be sent back for correction, and result in a delay in processing your claim.

3. Claims must be submitted within 30 days of receipt of your shipment.

*Note: ALL Damage claims require photo back up to be sent with them.

**Authorized returns must be in their original packaging and have NO PRICE STICKERS attached.

**Returns will not be accepted without a Return Authorization # (RA).
